ADOPTION APPLICATION
Welcome to the Toledo Animal Rescue and thank you for considering adopting a new pet from our rescue.
The information on this application is requested so that we may assist you in the selection of a new pet. The animal’s welfare is our foremost consideration. The consultation process is designed to help us determine if the adoption is in the animal’s best interest and to assist you in finding an animal most compatible with your lifestyle. In most cases, you will not be taking the animal home on the same day as you complete your application. Both you and the pet deserve our full consideration in this life-altering event and we want to take the time to process your application correctly, fairly and thoroughly. If you have other pets or children at home, we will ask you to schedule an appointment to bring them in to meet any potential new adoptees.
In order to protect our beloved canine and feline friends, please understand that we must conduct thorough reference and veterinarian checks to make sure that our animals will receive the loving homes they deserve. By submitting this application, you give the Toledo Animal Rescue permission to contact your veterinarian and/or landlord listed on your application (if applicable) and agree to the terms we have stated.

Please keep in mind that by completing an application, does not guarantee that you will be approved.
It is the policy of Toledo Animal Rescue that pets at home of the potential adopter must be spayed/neutered and current on all vaccines.
In order to be considered as an adopter today, you must...
Be 18 years of age or older;
Have identification showing your present address;
Have written consent from your landlord or park manager, if renting or living in a mobile home park;
Have satisfactory pet records of current pets
Our "guest list" changes frequently, so please revisit our website often. If you are interested in a specific age, breed, or type of animal, please ask our staff to add your name to our list. We will call if an animal that matches your criteria arrives at the Rescue providing your application has been approved.
Our facility features a special bonding room where you and your family can spend some quality time alone with our animals before deciding which one to adopt. If you already have pets, this is an excellent opportunity to see if they are compatible with the one you are considering adopting.
At Toledo Animal Rescue, we strive to find good matches between people and pets. Please note that Toledo Animal Rescue chooses the best appropriate adopter in the best interest of the animal, in some cases this does not mean the first application. In order to have a 'hold' on a dog or cat at Toledo Animal Rescue, the adopter must have an approved application. Toledo Animal Rescue reserves the right to refuse an adoption at any time.
